Every time you donate to charity you perform a special act of kindness and compassion that can make a real difference to someone in their time of need. Supporting the Pathways Telehealth nurse service, makes a difference to patients and families that receive specialist nurse expertise to help them understand their condition and navigate the health system, at no cost.

Our most recent service evaluation is complete and some highlights are:

  • 3,276 people have accessed the Pathways telehealth service
  • Pathways Telehealth Nurses made 23,711 assessments at an average of 7.75 per patient.
  • The average time per patient for an initial consultation and follow-up is 92.51 minutes.
  • Nurses are most commonly speaking with women(68.23%)
  • The majority of people who accessed the services were patients (75.52%) followed by next of kin or family members (24.48%)
  • Across all age groups, the highest number of people accessing the service were under the age of 18 along with a parent or next of kin (22.92%)
  • There has been an increase in access to the service from low to medium socioeconomic areas with 56.25% of participants coming from low to medium socioeconomic areas in the past 12 months, compared to 38.87% across the life of the program
  • There has been an increase in access to the service from regional and remote areas in the 2022 to 2023 reporting period with 43.75% of participants coming from regional or remote areas compared to 24.57% across the life of the program

How to reduce your income tax with donations

You can claim a tax deduction for donations of $2 or more, as long as these donations are made to an organisation that has been endorsed by the Australian Taxation Office (ATO) as a deductible gift recipient (DGR). The Centre for Community-Driven Research (CCDR) is a registered charity and has DGR status so your donation will be tax deductible.

CCDR developed the Pathways telehealth nurse program. It is a free service that anyone can access, regardless of their location. Pathways nurses conduct a holistic consultation to understand everything that is happening with the patient in front of them. It’s a model of care that helps people navigate the complex health system and access all that is available to them. The Pathways nurse then works with the patient to develop a care plan and offers support and guidance along the way. For people searching for a diagnosis, we have a special service to help them with that as well.
